Treatment Goals

To remove cellular debris or purulent exudate from the cavity, particularly in the presence of a skin bridge or sinus.

Aim of Dressing

Irrigate sinus cavities with iodine-based solution to reduce bacterial load and assist in infection control.

Post-operatively after washout

n the days following surgical washout, where residual cellular debris or purulent material may remain within the tract, ward-based iodine flushing may be performed prior to implementation of a long-term management plan.

Step 1

Prepare your sterile field

Prepare a sterile field and arrange all sterile products using aseptic technique.

Clean the wound

Cleanse the wound bed using normal saline and gauze.

Flush the sinus with iodine (part a)

Use appropriate PPE in case of inadvertent splashing during irrigation. Draw up the iodine into the 10ml syringe

Flush the sinus with iodine (part b)

ttach a 10 mL syringe to the 18G cannula catheter, ensuring the introducer is removed and safely discarded.

Flush the sinus with iodine (part c)

Advance the catheter into the sinus and gently flush the cavity by depressing the syringe. If required, gently open or augment the wound to improve tract access. Monitor the distal end for return of iodine solution.

Pack wound(s) with iodine

Soak gauze in Betadine, lightly squeeze to remove excess fluid, and place into the wound bed(s).

Secure

Cover the wounds with Zetuvit, followed by cotton bandage and crepe bandage.

Ongoing management

Once the acute infection has resolved, longer-term wound management can be initiated.
